Health Forms

• Please complete the Returning Student Registration Health Forms for all children.
• Please complete the PERMISSION TO ADMINISTER OVER THE COUNTER MEDICATION if you wish to grant your child permission to have over-the-counter medications administered at school. 
• Please complete the MEDICATION PERMISSION FORM if your child will have prescription medications dispensed in school. Please note: this includes inhalers and EpiPens. 
• FIELD TRIP MEDICATION FORM

All forms should be returned to the nurse at your child's school. 

Print copies are available in the main office at each school, as well as the nurse's office at each school.
