New State Immunization Requirements beginning in 2020-2021

Beginning with the 2020-21 school year, the meningococcal immunization (formerly Menactra, MenACWY and MCV4) will be required for:

Grade 7:  1 dose of MenACWY for all students

*Grade 11: 1 booster dose MenACWY received on or after 16 years of age (1 or more doses of MenACWY vaccine are acceptable as long as one dose was received on or after 16 years of age).

The meningitis B (MenB) immunization does not satisfy this requirement.

Please send in your student's most recent physical examination, which usually includes their immunization record.  Nurses review all immunization records for compliance and will contact parents or guardians if immunizations are missing.

 

Physical Examination Forms

This is a reminder that all new students and all students entering Kt,4th,7th and 10th grade are required by state law (105 CMR 220.000) to provide the school with a current (within the past 12 months) physical examination and documentation that all state required immunizations are up to date. Failure to provide the required immunization information could affect your child’s continued attendance in school.
