• Acceptable Use Policy: Our Acceptable Use Policy (or AUP) must be signed before a student is allowed to use LHS's technology facilities.
• Community Service Sign-Up Sign up for Community service
• Community Service Booklet An overview of the Community service program requirements
• Enrollment Update Use Use this form to change your address, or if you are entering or leaving Littleton Public Schools.
• Records Release Use this form if you need to transfer student records from a previous school district to Littleton.
• Library Notification Form Use this form to notify the LHS library when you are entering or leaving the school district.
• Medical Conditions Form Please fill out this form to let us know about any medical conditions which may require emergency care.
• New Student Checklist A complete list of records and forms you'll need to have before you start school.
• Over-the-Counter Medications Form This form is required in order for students to receive over-the-counter medication from the nurse. Use this form to list your student's prescription medications as well.
• Photo Release Form Please complete this form if you wish to prevent your child from being photographed, videotaped, or interviewed for use in school publications or outside media during the time he or she attends the Littleton Public Schools.
• Registration Form Each new student needs to complete and return this form.
